New problem (throttle actuator) 4.6iS....

Started a new thread as I thought this might warrant it, sorry.
The garage have repaired everything (see other thread) and all was running fine until I went to pick the car up! It now has a fault that puts the car into failsafe mode. Basically, turn the key and the engine starts in failsafe mode, the fault machine throws up this fault......
The mechanic can reset the fault using the diagnostic kit whilst the car is running and everything is fine, can even take it out for a drive and all is well, but as soon as you turn the engine off and restart the car the fault reappears.
The only thing that has been changed today is new PCV (rocker breather) hoses and it's had a wash. The mechanics are completely stumped and are going to try to get to the bottom of it tomorrow.
Just wondering if any one here had any ideas? Could it really be the throttle actuator? Could that have gone in the space of 20 minutes? Or is it an electrical fault some where (my guess)?
