Never had it done but what issues if any have you had with the insurance companies or are there particular types to go for which for some reason they don't worry about? What added premium do they generally charge?

Look for threads by Jason (BWChiptune). He can send you a tool too and you can do the car remap yourself, if you're not near Heathrow.

What gains can I expect on a 4.4V8?

Is it more about driveabilty and eliminating flat spots than actual gains?
