Looking for some guidance with my high milage 2003 e53 3.0D.
I've had this car for 5+ years and it has been great thus far.
I started by having low power in low rpm and exhaust fumes in the cabin. So looked under the hood and saw all the soot build up by the exhaust manifold. I bought a new one that was a bit wider than the original and replaced the old one.
Upon startup it was fine then I drove for 100m before the acceleration began surging and I could hear a hiss from the turbo. Smoke out the exhaust occurred when pressing accel too.
Have I damaged the boost controller or do y’all think it could simply be the exhaust gasket from the turbo is not seating right? I double checked the vacuum hoses and they are set right as per the image attached.

You say you bought and fitted a new one...new what?.. Manifold? as these on some models are made of stainless and are prone to cracking...mine had 7 cracks in it, so changed it for a cast one....have you run diagnostics?....could be the map sensor playing up which is if I recall correctley is fitted at the rear of the intake maifold by a 10mm bolt....

Yeah I replaced the exhaust manifold. The turbo was a pain to remove and enough worse reinstalling. The issue must be with something that occurred when I put it all back together because it was running waaaay better before. It’s currently not safe to drive in the road because it surges and smokes and you can clearly hear the turbo.
I’m guessing the pressure hose to the turbo was damaged or the vacuum lines but hopefully there are a few people who can suggest something else.

just thinking aloud here....

is it also possible that you may be getting low power due the the EGR unit is coked up & blocking air intake to the intake manifold....
I had this exact problem, the EGR unit is a nasty obstruction in the intake tube at best, let alone when it is a huge blob of soggy carbon goo partially restricting the air flow into the engine.
I fitted a K&N air filter, & replaced EGR Unit with a chunk if straight tube & blanked off the manifold....
Engine ran nicely straight after.

It be looking at the egr valve too , if its not closing it wont run right , a lot of carbon can buiñd up in them, once clean remove the vacuum line to it if it has one and plug it with a bolt and no more egr.

the internal obstruction of the egr is bad enough on its own.... let alone when the crud builds up on it.
