Very unusual tyre wear
I believe this may have been down to the tyres being low pressure, but this seems to have happened on the NSR tyre. I have a picture of the OSR picture as well. Does this warrant replacement, as thread is showing, but the tread is fine and not unevenly worn on both sides. The pressure on the bad tyre was about 10psi when filling it up - oops
If I was to replace, would it require the back two sets being replaced or will it have to be a full set?
Tread is around 6-7mm still on all tyres.

Hi I would get the tracking checked first , but the tyre needs to be replaced if threads showing like yours in the picture , unfortunately not only dangerous but illegal !
they recommend that you change all 4 to stop diff issues .

Mine was the same, could even be my photos! Although my tyres were nearer 3 mm when chord came through. The garage didn't mention an alignment check as all x5's squat on acceleration etc and it's a known trait. What he did say was the 26psi wasn't helping at all. Had them changed and now run maximum pressures as is the consensus that online forums suggest. 2.8bar rears 2.4bar fronts. That's on Dunlop sportmaxx 20" run flats. Wear after 1500miles looks good so far.

A well known trait of the X5, especially on 20" tyres , as mentioned if run on low pressure that part of the tyre would be in contact with the road..
Due to the large negative camber on the X5 the inner edges of the tyre take a hammering.
Very annoying when there is 6 mm left on the rest of the tyre, and recomendations that all 4 tyres are changed at the same time

I had the exact same wear on my rear 20in runflat tyres. You only need to replace the rear axle tyres not all four. I would seriously think about going to non-runflats. My 40D M Sport is like a completely different car getting rid of the run flats. I swapped all four for Kumho Crugen HP91 on paper they are equal to the Bridgestone run flats I removed but in real world driving they are quieter, last longer and grip better in the wet. I had them in my 30D MSport too. The bonuses....they are only £620 fitted for all four and you dont feel every pebble you drive over!!
I also got a full wheel alignment too to make sure everything was ok. If you are local in surrey you can drive mine to feel the difference.

Think we'll stick some run flats on as the car is in warrant but thanks for the advice. I can secure a full set for £800 anyway so think it won't be too bad. The ride is OK on run flats but would not want the transfer box risks etc. Thanks anyway!

No worries.
On another note, running non run flats will NOT void your warranty, mine still has another year to go on the warranty. However, pulling in the BMW garage with uneven tread wear on the same axle or different tyre pressure on the same axle MAY cause them to say your warranty is void on repairing your transfer boxs IF it breaks. Just make sure they are exactly the same sizes, speed rating and load rating.

No worries.
On another note, running non run flats will NOT void your warranty, mine still has another year to go on the warranty. The place that does my Hunter check (Protyre) recommends getting them checked every 6 months. They check for free, only charge if adjustment needed and you can watch the readings on a big TV monitor. They say if the alignment is ok, they will wear evenly.

Also just to add to this thread. When I bought mine and was looking on the Dunlop website, it actually states that new tyres can lose 1 lb of pressure per week naturally! I was aware of a little leakage, but if you don't check them at least fortnightly then money could be going out of your bank account sooner than you would like.
